November is your opportunity to make benefits changes for health insurance. If you wish to make changes to your health insurance coverage, you should complete the attached Group Insurance Enrollment/Change Form and return it to Human Resources no later than November 29, 2002. All changes will be effective January 1, 2003.

No New Benefits Handbook
Again this year we are not publishing a new Benefits Handbook as a cost savings measure. Carefully review the 2001 Benefits Handbook along with the 2002 Summary of Benefit Plan Changes before making your benefit election. (If you need a replacement 2001 Benefits Handbook or 2002 Summary of Benefit Plan Changes, please contact the Benefits Department.)
United Health Care PPO Directories
You may obtain updated United Health Care PPO directories at no cost to yourself or Airborne by calling United Health Care customer service at 1-888-350-5607. This information is also available on-line at www.myuhc.com. Remember it is still your responsibility to make sure that the provider you are using is participating in the network in order to receive the higher benefit level. If you are using a provider for the first time or if you are unsure if the provider is in the network, be sure to ask United Health Care.
Personal Benefits Statement
A personal benefits statement in a separate sealed envelope shows your current benefits coverage.
Government Notices
Federal law requires that we provide you with a notice about the benefits available under the health plan after a mastectomy. In addition, notice is provided about the claim determination and appeal process for the various plans and updated ERISA information is provided. You will find these notices attached.

Don’t Miss the Deadline!
You’ll need to make some important decisions by November 29, 2002. You will automatically receive your current election for medical, dental, and vision coverage for yourself (and any family members currently covered under your 2002 plan) unless you return the completed enrollment form by the deadline. If you are not making any changes you do not need to complete and return the enrollment form.
In order to participate in the Dependent Care Flexible Spending Account for 2003, you must submit a new enrollment form for 2003. If you fail to do so by November 29, 2002, you will not be enrolled in this plan. Dependent Care enrollment forms will be sent to current participants. Others may obtain the form upon request from the Benefits Department.
