Employee Monthly Contributions

A contribution for the employee only coverage is being introduced for employees. In addition, the monthly contribution for the various family levels is increasing.

Contributions will continue to be deducted from your paycheck on a pre-tax basis. This will reduce your taxable income for the year and lower your overall tax obligation.

When you enroll in the Enhanced PPO plan, the Enhanced Dental plan (includes orthodontia coverage) and the vision benefit is included.

1 Emergencies and accidents covered at 100% after $75 co-payment. Non-emergencies covered at 80% after $75 co-payment and satisfaction of $100 deductible.

2 Emergencies must be classified as life threatening. Accidents covered at 100% for first $500. Non-life threatening emergencies and non-accidents covered at 80% after the deductible.

This summary is intended to provide a brief overview of the changes for 2002. A more detailed description of the plan will be provided with the Open Enrollment materials. All changed are effective January 1, 2002.
