UnitedHealthcare is mailing updated insurance I.D. cards to all employees enrolled in the Enhanced PPO plan. Look for your card in the mail over the next week. If you need extra cards, you can request them by calling UnitedHealthcare at (888) 350-5607 or on-line at www.myuhc.com. You can also print temporary cards from www.myuhc.com. Employees enrolled in the HSA PPO and the Traditional Plan (pilots only) will not receive new cards unless a change in enrollment was made during open enrollment.

 

EyeMed will also be mailing new vision care I.D. cards to all covered employees. You also can request extra cards at www.eyemedvisioncare.com . Temporary cards are not available. As a new user to EyeMed you will have to register the first time you use the website.

 

MetLife, our dental carrier, does not issue I.D. cards. You should take a dental claim form with you when you visit the dentist. Claim forms are available online at the ABX Benefits website or in the forms bins near the Communication Centers.
