New United Healthcare Cards

United Health Care will begin mailing new insurance ID cards to enrolled employees in the next couple weeks. These cards replace the current cards for employees enrolled in the Enhanced PPO and HSA PPO plans. The new cards reflect the new Choice Plus network that is effective Jan. 1, 2006.

Employees needing additional cards can request the cards by calling (888) 350-5607 or by going on-line at www.myuhc.com. In addition, employees can print temporary cards on-line.

As a reminder, MetLife does not issue ID cards for the dental insurance.
