How to order replacement or extra UHC I.D. Cards

New United Healthcare I.D. cards have been mailed to all participating employees. The new cards should be used for medical service on or after Sept. 1, 2003. Employees who did not receive a card or who need extra cards may request replacements or additional cards by calling United Healthcare at (888) 350-5607.

Cards will be mailed to the employee's address of record as it appears on his or her paycheck. If the address is incorrect, the employee's supervisor should submit a change of address form (available on the e-mail bulletin board abx.forms.admin). Allow 1 to 2 weeks for the address update to reach the UHC system.

Here's how to order replacement or extra UHC ID cards:

1. Call UHC Customer Service 888-350-5607.

 IMPORTANT: Carefully listen to the prompts

2. Press 1 for information about medical claims networks and other info

3. Press 1 to be enrolled to get benefits

4. Enter your Social Security Number followed by the # sign

5. Press 1 for eligibility and to order I.D. cards

6. Enter your date of birth (MMDDYYYY)

7. Press 2 to order I.D. Cards

 

At any time after you have entered the new group number, press ZERO to speak with a customer service representative.
