Your EAP provides access to these and other helpful services:
Qualified referrals to experienced counselors
Up to 3 assessment visits with a counselor can be authorized at no cost to you. Additional visits are provided under your medical plan: If you see a network provider, your coverage is 100% coverage after a $25 co-payment for each additional visit if you are in the Enhanced PPO Plan, 80% coverage after a $25 co-payment for each additional visit if you are in the Value PPO Plan, or 80% coverage (after the deductible) if you are in the HSA PPO Plan. Other plan limits may apply.
Referral for child and elder care services
Financial counseling services for debt management and budget planning
Self-help groups such as Overeater Anonymous and Gamblers Anonymous and others
