Health Insurance (Medical, Dental & Vision)

Your health insurance programs continue through the last day of the month in which your employment ends. You have the right to continue coverage under COBRA for up to 18 months from the end of your coverage. You must pay the full premium plus a 2% administrative fee. Information about COBRA including the COBRA enrollment form will be automatically mailed to you when you terminate employment with ABX. You do not need to request it. A HIPAA Certificate of Coverage will be mailed to you directly by United HealthCare. The HIPAA Certificate of Coverage can help you obtain coverage with no pre-existing condition limitations depending upon the amount of time you were covered under the ABX Air plan. If you are part of a reduction in force and choose to separate with the company and receive a severance package, your benefits will continue through the last day of the month in which your severance pay ends if you return the proper paperwork.  Your COBRA forms will still be mailed to your home address and you must return the election form to start your COBRA, even if you are on severance.  If COBRA is elected, during severance you will pay only the active employee premiums.  At the start of the following month after your severance ends, you may choose to continue health coverage for the remaining of your 18 months of COBRA by sending in full COBRA payments.  

If you are part of a reduction in force and choose the layoff option as opposed to separating with the Company, you will be offered COBRA at the full COBRA premium.  Your forms will be mailed to your home address and you must follow the instructions on the form to receive the coverage.
