I.D. cards for Cole Managed Vision

As was recently announced during open enrollment, Cole Managed Vision will be administering the vision benefits for Airborne employees starting Jan. 1. Cole Managed Vision will be mailing I.D. cards to employees' home addresses after Jan. 5. It is not necessary to have the I.D. card to obtain the vision benefit; however, it will be easier for participating eye doctors to verify coverage with the I.D. card. Employees can also take a Cole Managed Vision Claim form to the eye doctor. These forms have been distributed to all locations. Employees using non-network eye doctors MUST take a claim form with them. When using out-of-network providers, employees will be reimbursed, up to the plan's limits, by the insurance company after a claim has been filed. Your eye doctor can verify coverage after Jan. 1 by calling 800-334-7591.
