7 a.m. - 9:30 a.m	Administration Bldg. (in parking lot)
10 a.m. – 11 a.m.	Logistics Services Parking Lot (Bldg. 10)
2 p.m. – 4 p.m.	Main gate (in front of Hangar 1005)

Cost: $9.00

Available to ABX employees & dependents (dependents must be 18 & older). Employees under age 18 need a release form signed by a guardian at the time of the flu shot. Employees need to show ABX ID.

Employees also may obtain a flu shot at Clinton Memorial Hospital's (CMH) Corporate Health Services after Dec. 11. Employees should make an appointment by calling (937)- 382-7785. Hours are 7 a.m. to 5 p.m. weekdays. The address is 2241 Rombach Avenue, Wilmington. Employees should go to CMH Corporate Health Center - not to the hospital.
